Abstract
- Background
Intercontinental air travel is recognized as a profound environmental, immunological, and physiological stressor that significantly compromises host respiratory health and increases susceptibility to acute upper respiratory infections, such as the common cold and influenza. Black elderberry (Sambucus nigra L.) has a long-standing history in traditional herbal medicine, supported by emerging observational and clinical studies, for its potential role as a supportive, immune-modulating phytotherapeutic agent against viral respiratory illnesses. This randomized, double-blind, placebo-controlled clinical trial involving 312 economy-class passengers traveling from Australia to an overseas destination was specifically designed to investigate whether a standardized membrane-filtered elderberry extract exerts beneficial effects on physical health—particularly respiratory outcomes—as well as mental well-being.
- Methods
A rigorous, randomized, double-blind, placebo-controlled clinical trial was conducted with 312 human participants undertaking long-haul intercontinental air travel. Incident cold episodes, the total duration of illness, and specific symptom severities were meticulously logged daily by participants using structured personal diaries and evaluated using the standardized Jackson score. Furthermore, participants completed three validated comprehensive surveys at baseline, immediately prior to travel, and four days post-travel, which included the Wisconsin Upper Respiratory Symptom Survey (WURSS-21) to capture respiratory symptom impact and the 12-Item Short Form Survey (SF-12) to assess health-related quality of life.
- Results
While the total number of acquired cold episodes was lower in the active treatment group compared to the placebo cohort (12 versus 17), this difference did not achieve statistical significance ($P = 0.40$). However, participants in the placebo group experienced a statistically significant longer duration of total cold episode days (117 days versus 57 days, $P = 0.02$), and the average cumulative symptom severity score over these illness days was also significantly higher in the placebo arm (583 versus 247, $P = 0.05$).
- Conclusions
These clinical data demonstrate a statistically significant reduction in both the duration and clinical severity of cold episodes among air travelers taking a standardized elderberry extract. Further targeted clinical research is warranted to substantiate these findings and comprehensively evaluate the broader physical and mental health benefits associated with elderberry supplementation.
